The VY plasty is a form of local flap plasty in which tissue is repositioned using a specific incision and suturing technique to achieve tissue gain in a particular direction. The name derives from the shape of the incision (a „V“) and the shape of the final suture (a „Y“). It is a fundamental technique for extending the skin or relieving tension in areas under strain by repositioning tissue.

The ingenious principle: how a „V“ becomes a „Y“

The concept behind VY plasty is easier to understand than it sounds. It is a surgical technique based on geometry:

  1. The V-neck The surgeon makes a V-shaped incision in the skin. The tissue within this „V“ forms a triangular skin flap.
  2. The postponement This V-shaped flap is carefully detached from the underlying tissue so that it becomes mobile. The entire flap is then pushed forwards towards the tip of the V.
  3. The Y-clamp As the flap moves forwards, a gap is created at its original base. This gap is then sutured directly, forming the vertical line of the „Y“. The two legs of the original „V“ form the arms of the „Y“.

The net result: the fabric at the tip of the original „V“ has been shifted forwards, resulting in an increase in length or volume at that point.

Aesthetic procedure: the lip lift for lip augmentation

The V-Y plasty is frequently used to emphasise the red part of the lips from the inside by extending the mucous membrane (known as a „lip lift“). How it works: The surgeon makes one or more V-shaped incisions on the inside of the lip (on the moist mucous membrane). The V-Y flap procedure involves moving the inside of the lip outwards and forwards. Result: More of the natural lip colour becomes visible, and the lips appear fuller and more sensual without the use of foreign materials (such as fillers). The scars are inside the mouth and are therefore invisible.

Reconstructive application: Scar correction

In cases of tight, constricting scars (scar contractures), which often develop following burns or injuries, VY plasty can work wonders. How it works: The scar, which is causing the tissue to contract, is cut away. A V-shaped flap is then created from healthy neighbouring tissue and inserted into the area where the scar was removed. Result: This technique introduces healthy, supple tissue into the defect and lengthens the shortened area. Tension is relieved, mobility (e.g. in joints) is improved, and the appearance of the scar is significantly reduced.
